Linda van Loon, RN, BSN

Linda S. van Loon, RN, BSN, is the Care Coordinator for Parkview Medical Center's new unit called the Transitional Care Center. Linda received her LPN in 1978 and her BSN in 1987 from Colorado State University in Pueblo, Colorado. Linda's background includes over 18 years in acute medical surgical, cardiac, post cardiac surgery, rehabilitation units and the Home Health Care field. Her passion can be found in assisting patients and families find success with their recovery, and self-care once patient return to their homes. Linda has also worked in the area of Early Child Care centers, homes and in-school settings as a Nurse consultant, and as a member of a community board for adult protection.

In 2014, Parkview Medical Center realized that it needed to reduce its readmission rate significantly while also fixing a communication gap between home health care agencies and other post-acute facilities. To address these issues, my director and a team of other leaders joined together to pilot a new transition care center that went live in 2016.
