However, as attendees discussed during MEDITECH’s recent RHTP Office Hours session on July 21, 2026, securing and managing these grants requires much more than simply demonstrating local clinical need. Reviewers look for strategic, data-supported proposals that demonstrate clear outcome metrics, support cross-community collaboration, track clinical performance, and improve community health.
To support our customers across all stages of the grant cycle and program deployments, MEDITECH launched the MEDITECH RHTP Resource Center. Located in our customer service portal, the centralized hub delivers materials that provide organizations with a clear framework to outline their EHR-driven strategies and turn federal investment into lasting rural transformation. We designed these materials to help our customers maximize their Expanse investment by using existing tools to support RHTP funding requirements and reduce clinician burden.
Understanding Guardrails
Before drafting a proposal, organizations must navigate the spending limits and funding distribution models established by the Centers for Medicare & Medicaid Services (CMS). CMS divides the total annual funding pool ($10 billion per fiscal year through 2030) between a fixed base allotment, which provides $100 million annually to every state, and a variable pool that factors in rurality metrics, financial distress, and application technical scores.
- Strict Spending Limits: Uncompensated care expenditures are capped at 15%, and internal IT spending is limited to a 5% cap unless tied directly to consumer-facing tools for chronic disease management.
- State-by-State Variation: Funding mechanisms vary dramatically by state. While some states utilize regional "hub-and-spoke" models (like Missouri's ToRCH Hub Anchor initiative), others are prioritizing direct technology grants, paramedicine/home-health integrations, or behavioral health access points.
- Phase Alignment: A poll during the July 21 Office Hours revealed that 61% of participating organizations are already in the "Submission & Beyond" phase, seeking operational support to monitor awarded programs, while 39% remain in the active planning or drafting stages.

- Behavioral Health, OUD/SUD, and Suicide Prevention: Using the Depression Screening and Suicide Prevention Toolkit or the Opioid Stewardship Toolkit, facilities can monitor universal risk screenings and automate naloxone co-prescribing alerts to reduce overdose rates.
Sustainable Access
- Sustainable Care Models: To help lower the cost of care, healthcare organizations can use Traverse Exchange to share data between facilities to manage chronic conditions and prevent readmissions. MEDITECH’s Business and Clinical Analytics (BCA) provides benchmarking and analytics, such as tracking Traverse query completion rates or the percentage of high-risk patients enrolled in patient registries.
Workforce Development
- Improving Efficiency: Solutions such as ambient listening and AI documentation optimize workflows and significantly cut down on the administrative “pajama time” to increase EHR satisfaction benchmarks and decrease clinician burnout rates.
Technology Innovation
- Telehealth & Virtual Care: Expand remote patient monitoring (RPM) and virtual care via the MyHealth patient portal to lower 30-day readmissions. Additionally, facilities are tapping into mobile telemetry and paramedicine programs
Innovative Care Models
- Value-Based Care & APMs: To drive higher performance scores on specific payer metrics and lower the cost of care, facilities can use Quality Vantage to monitor performance scores alongside Care Compass registries to track Annual Medicare Wellness Visit completion rates and manage chronic disease cohorts.
Winning Strategies for Grant Writing: Expert Advice from the Field
During the Office Hours session, Cassie Robbins-Forbus, Grant Writer for Samaritan Medical Center, shared high-level, practical recommendations for navigating federal and state funding streams:
- Start Early and Plan Backwards: Map every deadline backwards from the submission date. Account for registration lead times on platforms like SAM.gov, Grants.gov, or state vendor portals (which can take weeks).
- Follow Instructions to the Letter: Applications must follow the strict formatting rules set for fonts, margins, page limits, and exact attachment names. Non-compliance or a blank attachment can cause an application to be disqualified immediately without review.
- Write Plainly & Mirror Funders’ Language: Answer every prompt directly and reuse key phrases from the Notice of Funding Opportunity (NOFO). Keep responses self-contained per question, as reviewers often evaluate individual sections in isolation without seeing the entire package.
- Lead with Your Story, Driven by Data: Ground your community narrative in concrete metrics. Utilize data from your Community Health Needs Assessments (CHNA) alongside EHR registries to define measurable outcome goals.
- Build a Budget That Tells a Story: Ensure financial figures are exact and backed by actual vendor quotes. A precise, transparent budget establishes credibility during application scoring and prevents post-award auditing headaches.
- Look Beyond the RHTP: Explore complementary rural funding streams through HRSA (Small Health Care Provider Quality Improvement), SAMHSA (Opioid Response/SUD grants), the FCC (telecom and broadband support), and the USDA (facility loans and grants).
