Chief Operating Officer Darlene Cunha, MMHC, BSN, RN, ACHE, at CharterCARE Health Partners in Providence, Rhode Island discusses her organization’s journey to high reliability. Cunha talks about how CharterCARE achieved a 40 percent reduction in hospital-acquired conditions, 47 percent reduction in catheter-associated UTIs, 81 percent fewer falls, and 14 percent drop in readmission rates.
